Job summary

Allina Health Cambridge Medical Center is seeking a part-time RN Hospital Care Coordinator to join our System CM Hospital Care Management team. The role supports 8-hour day shifts with 16 hours per two-week pay period and occasional weekend rotations.

The RN Care Coordinator assesses, plans, and intervenes to coordinate care for medically complex patients as they transition from hospital to home or other settings. Strong collaboration with providers and families is essential.

Qualifications

• Bachelor's degree in nursing.
• 2+ years of nursing experience (clinical) or RNCC/case management experience preferred.

Responsibilities

• Provides clinical coordination services including assessment, planning and intervention for medically complex patients.
• Ensures timely progression of care and identifies barriers to discharge.
• Collaborates with interdisciplinary teams, patients and families for safe transition from hospital.
• Supports palliative and advanced care planning discussions with patients and families.
• Coordinates discharge planning and post-discharge care with community resources.
