Job summary

Responsibilities include reviewing records for medical necessity, collaborating with the interdisciplinary team, and protecting resident confidentiality under HIPAA. Requires a valid LVN license and CPR/BLS.

Qualifications

• 1 year case management experience in skilled nursing preferred
• 5+ years as a licensed nurse
• Excellent verbal and written skills
• Knowledge of Point Click Care helpful

Responsibilities

• Manage all types of insurance cases from admission through discharge
• Follow up with residents’ post-discharge follow up
• Communicate resident status and care plan changes to payer
• Document payer interactions regarding resident progress and outcomes
• Coordinate with facility team to ensure total resident care regimen
• Collaborate with business office to ensure accurate census and payment
