Job summary

Oak Street Health is seeking a Registered Nurse (Spanish Preferred) in Brooklyn, NY. The RN will coordinate patient care and ensure a seamless experience for patients and their support teams. This role emphasizes clinical assessment, education, and collaboration within the care team to improve patient outcomes.

You will work with standardized protocols, perform phlebotomy and point-of-care tests as needed, and engage patients in preventive care to reduce ED visits and hospitalizations.

Qualifications

• Active RN licensure in good standing.
• BLS certification.
• Experience with Electronic Medical Records (EMR).
• Spanish language proficiency is strongly preferred.
• Authorized to work in the US.

Responsibilities

• Provide competent nursing care in a safe, consistent manner.
• Respond to incoming telephonic requests and triage patient needs.
• Perform triage and symptom management for remote or in-person patients.
• Review and manage patient medications and refills.
• Educate patients and care partners on chronic conditions.
