Job summary

Qualifications

• Must hold RN license with active, unrestricted license in the state of employment.
• Current BLS certification.
• Minimum 2 years of RN experience in a physician's office, clinic or hospital setting.
• Knowledge of chronic diseases (COPD/ asthma, diabetes, CHF, IHD).
• TB screening and immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, Pertussis required.
• Fluent English and Spanish language skills preferred.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Support longitudinal care and patient education, medication reconciliation, Motivational Interviewing and goal setting.
• Coordinate transition of chronic care patients from inpatient to outpatient settings.
• Document accurately in the electronic medical record.
• Participate in PCC meetings and weekly reports.
• Maintain ongoing competency with evidence-based practices.
• Perform other related duties as assigned.
