Job summary

The role focuses on obtaining prior authorizations, collaborating with the care team, and ensuring safe transitions while improving LOS, patient satisfaction, and outcomes. A BSN preferred; TN RN license required; 3+ years acute care experience preferred.

Qualifications

• Bachelor of Science in Nursing preferred; ADN/ Diploma acceptable.
• Current licensure as Registered Nurse in Tennessee.
• ACM/CCM or related case management certification within 3 years of hire preferred.
• Minimum 3 years of acute clinical care experience preferred.
• Strong communication and relationship-building skills.
• Knowledge of care coordination and discharge planning.

Responsibilities

• Obtains pre-certifications and authorizations from third-party payers prior to hospitalization.
• Facilitates continued stay and post-acute service authorizations per requirements.
• Documents and analyzes clinical denials and supports action plans.
• Maintains knowledge of care management regulations and utilization review.
• Leads transition of care planning across levels of care with the team.
• Communicates with outpatient providers to coordinate continuity of care.
• Drives performance improvement and efficient resource use.
