Qualifications

• Associate's degree in nursing is required.

• Current RN license in Tennessee and BLS certification.

Responsibilities

• Assesses physical, psychological and social needs and improves care for patient and family.
• Creates a seamless flow through the health system to facilitate care.
• Develops and coordinates education, resources and activities for patients and families.
• Maintains reliable systems to document, track and monitor patient data.
• Performs nursing procedures within scope of practice and clinical training.
• May perform telephone duties within RN role.
• Performs other job functions as assigned or requested.
