Community Care is hiring an RN Care/Case Manager for Kenosha County. The full-time role runs 8:00 a.m. to 4:30 p.m., with an $8,000 sign-on bonus and comprehensive benefits. You will collaborate with care teams to deliver consistent care and manage assessments and documentation to support member needs.

The position requires an active Wisconsin RN license and 2+ years of experience in care or case management for frail or elderly populations. This is a day-shift role based in Kenosha County.

Qualifications

• Must have 2+ years of personal, clinical leadership, or care/service experience with frail or elderly populations
• Current Wisconsin RN license in good standing is required
• 2 years of recent case management preferred

Responsibilities

• Collaborate with site management to ensure uniform care delivery for Community Care members
• Conduct phone and in-person assessments to identify health needs
• Complete documentation and maintain member records
