Job summary

Community Care is hiring an RN Care/Case Manager for Kenosha County. The full-time role runs 8:00 a.m. to 4:30 p.m., with an $8,000 sign-on bonus and comprehensive benefits. You will collaborate with care teams to deliver consistent care and manage assessments and documentation to support member needs.

The position requires an active Wisconsin RN license and 2+ years of experience in care or case management for frail or elderly populations. This is a day-shift role based in Kenosha County.

Qualifications

• Must have 2+ years of personal, clinical leadership, or care/service experience with frail or elderly populations
• Current Wisconsin RN license in good standing is required
• 2 years of recent case management preferred

RN Care/Case Manager, Full-Time

Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Kenosha County.

RN Care/Case Manager Job Responsibilities:

• Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs.
• Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs
• Complete all required documentation and charting and assists in the development and maintenance of member records

Job Requirements:

• Experience: Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
• Licensure: A current Wisconsin Registered Nurse license in good standing

RN Care/Case Manager Job Schedule:

• 8:00 a.m. - 4:30 p.m., 40 hours a week
