Job summary

Community Care is seeking a Registered Nurse (RN) for the Family Care Program in Milwaukee, WI. The position is Monday through Friday, 8:00 am to 4:30 pm, with a $10,000 sign-on bonus.

You will manage caseloads, complete comprehensive assessments, and develop member-centered care plans in collaboration with a multidisciplinary team. Ideal candidates hold an Associate's Degree in Nursing, Wisconsin RN license, and 3 years of experience with older adults or disabilities; a Bachelor's in Human

Qualifications

• Must hold Wisconsin RN license.
• Minimum of 3 years experience with older adults or disabilities in nursing.
• Bachelor's degree in Human Services or related field preferred.

Responsibilities

• Manage caseload of Family Care Program members.

• Travel locally as required with valid driver's license and insurance.

Registered Nurse (RN) Job Responsibilities:

• Manage caseload of Family Care Program members
• Complete comprehensive clinical assessments
• Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
• Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Registered Nurse (RN)Job Requirements:

• Education:Associate's Degree in Nursing Required; Bachelor's degree in Human Services or closely related field; preferred.
• Licensure: WisconsinRegistered Nurse/RN license required.Current long-term care functional screener certification preferred (eligibility for this certification is required)
• Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required. 3 years of work in community nursing preferred.

RN Nurse Work Hours - Monday through Friday position, 8:00 am to 4:30 pm.
