Job summary

Community Care is seeking an RN Care/Case Manager in Mount Pleasant, Wisconsin. This full-time role supports our aging and disabled population with coordinated care, home-based services, and timely access to appointments.

The position offers an 8:00 a.m. - 4:30 p.m. schedule, eligibility for a $8,000 sign-on bonus, and a comprehensive benefits package. Wisconsin RN license and at least 2 years of relevant care or case management experience are required.

Qualifications

• Minimum 2 years of care or service for frail or elderly population.
• Current Wisconsin Registered Nurse license in good standing.
• Experience in personal, clinical leadership or case management preferred.

Responsibilities

• Collaborate with site management to ensure consistent care delivery for Community Care members.
• Conduct phone and in-person assessments to identify health needs.
• Complete documentation and charting and maintain member records.

Job Description

RN Care/Case Manager, Full-Time Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Racine County.

RN Care/Case Manager Job Responsibilities:

• Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs.
• Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs
• Complete all required documentation and charting and assists in the development and maintenance of member records

Job Requirements:

• Experience: Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
• Licensure: A current Wisconsin Registered Nurse license in good standing

RN Care/Case Manager Job Schedule:

• 8:00 a.m. - 4:30 p.m., 40 hours a week
