Job summary

Community Care is seeking a Registered Nurse (RN) for the Family Care Program in Milwaukee, WI. The position is Monday through Friday, 8:00 am to 4:30 pm, with a $10,000 sign-on bonus.

You will manage caseloads, complete comprehensive assessments, and develop member-centered care plans in collaboration with a multidisciplinary team. Ideal candidates hold an Associate's Degree in Nursing, Wisconsin RN license, and 3 years of experience with older adults or disabilities; a Bachelor's in Human

Qualifications

Must hold Wisconsin RN license.
Minimum of 3 years experience with older adults or disabilities in nursing.
Bachelor's degree in Human Services or related field preferred.

Responsibilities

Manage caseload of Family Care Program members.
Complete comprehensive clinical assessments.
Develop member-centered care plans and provide ongoing care coordination and education.
Travel locally as required with valid driver's license and insurance.
