Job summary

Community Care is seeking an RN Care/Case Manager for the Day Shift in Outagamie County, Appleton, WI. This full-time role includes a $8,000 sign-on bonus and opportunities to coordinate care for older adults and adults with disabilities.

You will perform assessments, develop and maintain member records, and collaborate with the site management team to ensure consistent care delivery. Wisconsin RN license in good standing and at least 2 years of care or case management experience are required.

Qualifications

• 2 years of personal, clinical leadership, and/or employment experience providing care for a frail or elderly population.
• Wisconsin Registered Nurse license in good standing.

Responsibilities

• Collaborate with site management to ensure uniform care delivery for Community Care members, including problem solving for appointments, transportation, and home care needs.
• Conduct phone and in-person assessments to identify immediate and potential health needs.
• Complete required documentation and maintain member records.
