Job summary

RHA Health Services is seeking a CAP Nurse Manager (RN) in the Wilson area to ensure health, safety, and well-being of program participants. The role collaborates with CAP Social Worker, families, healthcare providers, and service providers to coordinate high-quality, person-centered care.

The RN will conduct assessments, develop care plans, coordinate services, monitor delivery, and support Medicaid-compliant documentation.

Qualifications

• RN license in North Carolina with a 2- or 4-year degree.
• One year of case management experience in home care, long-term care, personal care, or related work.
• Completion of an NC Medicaid-certified training program within 90 calendar days of employment.
• Bachelor’s degree or nursing license with minimal experience may be designated as an apprentice and supervised.

Responsibilities

• Assess beneficiaries and families to evaluate medical, psychological, behavioral, and social needs.
• Develop and maintain individualized, person-centered care plans.
• Coordinate services to meet the care plan and Medicaid requirements.
• Monitor and document beneficiary progress and adjust plans as needed.
• Provide counseling and support to families navigating challenges and services.
• Maintain compliant records and ensure program requirements are met.
• Collaborate with multidisciplinary teams and advocate for participants.
