Job summary

Socket.dev is seeking an RN Care Manager in the OKC/Norman area. You will deliver integrated nursing and case management for individuals with behavioral health, substance use, and medical needs.

Responsibilities include assessments, nursing care, care coordination, and health education in collaboration with interdisciplinary teams. The role emphasizes trauma-informed care, documentation, and community resource linkage to support recovery and improved health outcomes.

Qualifications

• RN with Oklahoma license required; ODMHSAS Case Manager II within 60 days.
• ADN required; BSN preferred with 6+ months BH/SUD experience for BSN.
• Must have a valid driver license and insurable driving record.

Responsibilities

• Provide nursing care to individuals with serious mental illness and substance use disorders.
• Deliver case management services and coordinate with treatment teams.
• Conduct and document nursing assessments and medication administration.
• Educate on health, medications, relapse prevention, and wellness activities.
• Follow up after hospital or ED visits and monitor engagement and treatment plans.
• Document interventions per policy and participate in quality improvement.
• Assist clients in accessing community resources for stabilization and recovery.
