Job summary

NorthCare in Oklahoma City is seeking a registered nurse or licensed practical nurse to identify, screen, plan, track, and coordinate integrated care for consumers with co‑morbid mental illness and addiction disorders. You will develop and supervise personalized care plans and conduct comprehensive nursing assessments.

The role requires strong teamwork with an interdisciplinary treatment team, motivational interviewing, advocacy for clients and families, and linking to community resources.

Qualifications

• Must possess and maintain a current and active license to practice as a Registered Nurse or Licensed Practical Nurse.
• Participate in ODMHSAS training related to their discipline.
• Prefer a minimum of one year experience in a mental health-related field or healthcare setting.
• Proficiency in MS Word and intermediate computer skills; strong written and verbal communication.

Responsibilities

• Identify, screen, plan, track, and coordinate integrated care for consumers with co-morbidities including mental illness and addiction disorders.
• Provide comprehensive nursing assessment to gather physical and behavioral health information.
• Advocate for consumers and families, linking to care team and community resources.
• Lead interdisciplinary treatment team meetings to review assessments and update care plans.
