Job summary

Family & Children’s Services in the OKC/Norman area seeks an RN Care Manager to deliver integrated nursing and case management services for individuals with behavioral health, substance use, and medical needs. You will assess, provide nursing care, coordinate care, and educate clients to support continuity of care and recovery.

The role collaborates with interdisciplinary teams and community providers to improve health outcomes and ensure safe treatment transitions.

Qualifications

• ADN required with clinical nursing experience in behavioral health or SUD.
• BSN preferred with some related clinical experience.
• Must hold Oklahoma RN licensure.

Responsibilities

• Provide professional nursing care to individuals with behavioral health and co-occurring conditions.
• Provide case management services and coordinate care with the treatment team.
• Conduct and document nursing assessments and update care plans.
• Administer and monitor prescribed medications as ordered.
• Provide nursing education on health topics including relapse prevention and wellness.
• Support transitions between care settings and accompany clients to appointments as needed.
• Maintain accurate documentation per policy and regulatory standards.
• Participate in quality improvement and outcome monitoring activities.
