Job summary

Fundamental is seeking an RNAC (MDS Coordinator) to oversee patient/resident care from admission to discharge under the supervision of the Director of Nursing. The role centers on coordinating assessments, developing care plans, and ensuring quality outcomes.

The RNAC must hold a current RN license in the applicable state and bring 2+ years of nursing and LTC experience, with ICD-10 coding proficiency. A $10,000 sign-on bonus for full-time hires is offered, plus $5,000 for part-time, payable

Qualifications

• Must have a current, unencumbered RN license in state of practice.
• Minimum 2 years of direct hands-on nursing experience.
• Minimum 2 years of long-term care experience.
• Preferably at least 1 year of RAI experience (focus in MDS).
• Proficiency in ICD-10 coding.

Responsibilities

• Identify and monitor resident skilled services.
• Coordinate the RAI process.
• Complete an accurate MDS and develop the interdisciplinary Plan of Care.
• Monitor patient/resident status for changes and barriers to discharge.
• Participate in daily PPS Interdisciplinary Team Meeting (ITM).
• Collaborate with nursing management and healthcare team.
