Job summary

Chestnut Hill Lodge Health and Rehabilitation Center in Wyndmoor, PA is seeking an RN MDS Coordinator / RNAC to oversee MDS assessments, care planning, and regulatory compliance. This Mon-Fri position offers a meaningful role in a 181-bed facility.

As part of the interdisciplinary team, you will educate residents and families, ensure accurate documentation for PDPM, Medicare and Medicaid, and support audits to sustain high-quality resident care.

Qualifications

• Prior experience as an RNAC or MDS Coordinator required.
• Current RN license in Pennsylvania.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Oversee MDS assessments for admissions and ongoing resident updates.
• Develop and update individualized care plans with the interdisciplinary team.
• Ensure documentation supports Medicare, Medicaid, and PDPM reimbursement.
• Monitor quality measures and support facility improvement goals.
• Assist with audits and surveys by preparing required documentation.
