Job summary

Agrace is seeking a Registered Nurse for Alt Shift to support home-based care for a Southwestern Wisconsin service area. The role emphasizes patient- and family-centered nursing, care coordination, and flexible start times with 8- or 12-hour shifts.

Qualifications

• Current, unrestricted RN license in state of employment.
• One-year nursing experience preferred.
• Valid driver’s license, auto insurance, and reliable vehicle.
• Must be at least 18 years of age.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics and standard precautions.

Responsibilities

• Assess patients and families for physical, pain, psychosocial, emotional, spiritual, and educational needs per the plan of care.
• Collect data from patient, family, and inter-disciplinary team; document findings.
• Identify and document assessment data from nursing interview and exams.
• Report significant changes in condition to IDT and manage care communications.
• Collaborate with IDT to create care plans and hospice eligibility.
