Job summary

Agrace, a leading Wisconsin hospice, seeks an RN for Alt Shift in Dodgeville. This 0.9 FTE night shift role supports homecare families, conducts assessments, plans care, and coordinates with the Interdisciplinary Team to ensure compassionate, patient-centered care.

You’ll supervise CNAs/LPNs, provide education, document in EHR, and participate in on-call rotations and holidays. A valid RN license, at least 18, and a vehicle are required; a $10,000 sign-on bonus accompanies a competitive benefits

Qualifications

Current, unrestricted Registered Nursing License in state of employment.
One year nursing experience preferred.
Valid driver’s license, auto insurance, and access to a reliable vehicle.
Must be at least 18 years of age.
Basic Microsoft Office and intermediate EHR skills.

Responsibilities

Assess patients and families to address physical, pain, psychosocial, and educational needs.
Conduct patient visits, provide interventions, and manage pain and symptoms.
Develop and revise Plan of Care with the IDT and family.
Document outcomes and changes in the EHR; delegate visits as needed.
Precept new staff and students; oversee CNA/LPN work.
