Job summary

Agrace in Baraboo, WI is hiring an RN Alt Shift for 32 hours per week on the night shift. This role includes on-call rotation, weekend and holiday coverage, and a $10,000 sign-on bonus.

You will perform assessments, care planning, and case management to support hospice patients and their families. Responsibilities include patient and family assessments, visits, and collaboration with the IDT to manage plan of care; CNA/LPN oversight and documentation in the EHR system.

Qualifications

• Current, unrestricted Registered Nursing License in state of employment.
• One-year nursing experience, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Must be at least 18 years of age.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsibilities

• Assess patients and families to identify physical, pain, psychosocial, emotional, spiritual, and educational needs.
• Provide interventions, treatments, and pain management; evaluate and adjust as needed.
• Develop and revise the plan of care with the Interdisciplinary Team and family; document outcomes.
• Oversee CNA/LPN staff, delegate visits, and participate in staff meetings.
