Job summary

Agrace is seeking a Registered Nurse for the Alt Shift (NOC) in Baraboo, WI. This 0.7 FTE role covers a 12:00 am to 8:30 am shift with on-call rotation, weekends (every other weekend) and three holidays per year. Sign-on bonus of $10,000 is included.

You will perform patient and family assessments, implement the plan of care, provide interventions and pain management, and document in the EHR, coordinating with the interdisciplinary team to maintain high-quality hospice care.

Qualifications

• Current, unrestricted RN license in state of employment.
• One-year nursing experience preferred.
• Valid driver’s license and access to a reliable vehicle.
• Must be at least 18 years old.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics and safe patient handling.

Responsibilities

• Assess patients and family needs as outlined by the plan of care.
• Collect data from patient, family, and care team.
• Document assessment data and changes in the nursing interview.
• Report significant changes to IDT and primary care.
• Collaborate to determine hospice eligibility.
• Provide patient and family education on pain, symptoms and end-of-life care.
