Job summary

Agrace is seeking a skilled Registered Nurse to join our Alt Shift team in Wisconsin. This role covers 0.8 FTE (32 hours/week) on the PM shift, with weekdays, evening hours (16:00–00:30), and on-call rotations including one NOC shift per month, every other weekend, and a few holidays.

Qualifications

• Current, unrestricted Registered Nursing License in state of employment.
• One-year nursing experience, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Must be at least 18 years of age.
• Basic Microsoft Office skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsibilities

• Conduct patient and family assessments and document findings.
• Provide nursing interventions, pain management, and care planning.
• Develop and revise the plan of care with the IDT and family.
• Oversee care across homecare teams; delegate visits as needed.
• Participate in meetings and precept new staff.
