Job summary

Aging Well Eldercare in the Maryland suburbs seeks a registered nurse to join a relationship-based care team serving DC-area clients. You will conduct assessments, develop care plans, visit clients, and liaise with families and providers to ensure coordinated care.

The ideal candidate holds an MD RN license with DC a plus, and CCM/CMC certification is a bonus. Travel within the DC metro area is required; flexible scheduling supports work-life balance.

Qualifications

• RN license required (MD); DC license a plus.
• CCM or CMC certification is a plus.
• Experience with older adults, home health, hospice, or related fields.

Responsibilities

• Conduct client assessments and develop care plans.
• Visit clients and support ongoing care needs.
• Communicate regularly with families and providers.
• Provide guidance during times of transition or change.
• Occasional weekend on-call support with eligible bonus pay.
