Job summary

Maryland Care Management, Inc. (MCMI) is seeking a Care Manager to plan, develop, implement, and oversee care plans for high-risk members, coordinating services and resources to improve accessibility and appropriate utilization.

The role collaborates with physicians and interdisciplinary teams, conducts assessments, promotes continuity of care, and maintains records. Flexible work arrangements and comprehensive benefits are offered to support diverse patients and staff.

Qualifications

• Graduate of an Accredited School of Nursing.
• Current, unrestricted RN license to practice in Maryland.
• 2+ years of clinical nursing experience in an acute care or community setting.
• 2+ years of Care Management experience.
• Experience assisting members with accessing community agencies and resources.
• Knowledge of government-sponsored managed care programs is preferred.
• BSN preferred.
• CCM preferred.

Responsibilities

• Develop, maintain, and monitor processes to promote timely identification and assessment of high risk and special needs members.
• Interdisciplinary team member in management of identified members, collaborating with physicians to determine medical needs.
• Conduct telephone or face-to-face interviews to identify risk factors and care management needs.
• Foster networks with service and resource providers to ensure continuity and quality of care.
• Initiate and implement care plans to deliver quality care.
• Use clinical judgment to discharge members from care management per policies.
• Maintain care management records and data integrity in health plan systems.
• Collect, evaluate, and report clinical, functional and program activity as applicable.
• Identify cases to assess impact of care management services and assist with data collection.
• Educate medical providers about members' special needs for treatment planning.
• Coordinate with community support and social service systems for members.
• Reduce care costs without sacrificing quality through coordination and collaboration.
• Collaborate with multidisciplinary teams to facilitate member care and reduce barriers.
• Assist with development of care management policies and procedures.
• Communicate with supervisor regarding departmental issues, member issues, program status, and compliance.
