Job summary

Aging Well Eldercare in the Maryland suburbs seeks a registered nurse to join a relationship-based care team serving DC-area clients. You will conduct assessments, develop care plans, visit clients, and liaise with families and providers to ensure coordinated care.

The ideal candidate holds an MD RN license with DC a plus, and CCM/CMC certification is a bonus. Travel within the DC metro area is required; flexible scheduling supports work-life balance.

Qualifications

• RN license required (MD); DC license a plus.
• CCM or CMC certification is a plus.
• Experience with older adults, home health, hospice, or related fields.

Responsibilities

• Conduct client assessments and develop care plans.

In this role, you will:

• Conduct client assessments and develop care plans
• Visit clients and support ongoing care needs
• Communicate regularly with families and providers
• Provide guidance during times of transition or change
• Provide occasional weekend on-call support on an occasional basis, with eligibility for bonus pay

We’re looking for someone who:

• Has experience with older adults, home health, hospice, or related fields
• Is highly organized and comfortable managing multiple moving parts
• Is comfortable using technology (e.g., Google Workspace, databases)
• Builds strong relationships and communicates thoughtfully
• Demonstrates emotional intelligence and healthy boundaries

Requirements:

• RN license (MD required; DC a plus), CCM or CMC certification a plus
• Ability to travel for in-person client visits in the D.C. Metro area
