Job summary

National Care Advisors in the DC/Baltimore area seeks a Nurse Case Manager to coordinate case management for special needs and eldercare clients, enhancing quality of life while collaborating with families, attorneys, planners and trustees.

The role supports client goals through timely documentation, care team coordination and proactive communication with caregivers. This is a salaried, remote-friendly position with competitive benefits and opportunities to travel regionally.

Qualifications

• BSN required; CCM or CRRN preferred
• Minimum of 5 years clinical nursing experience; field case management or workers’ compensation case management preferred
• Experience with third party benefits – health insurance, Medicaid, Social Security, Developmental Disability services
• Excellent written and verbal communication skills
• Proficient in using MacOS and MS Office
• Ability to quickly learn and adapt to new technologies and tools in a Mac environment
• Self-motivated, organized and able to work independently in a virtual environment
• Strong collaboration skills and ability to resolve conflicts
• Ability to research and develop solutions to client challenges
• Excellent customer service skills
• Overnight travel regionally 1-2 times per month and occasional nationwide travel
• Willingness to meet timely documentation requirements

Responsibilities

• Provide case management services responsive to special needs and eldercare client care and quality of life challenges.
• Collaborate with the primary family caregivers, attorneys, financial planners and trustees serving the client.
• Maintain timely documentation and coordinate with care teams to support client goals.
