Job summary

Chapters Health System in Florida seeks an Admission RN to assess patients in residences or hospital settings and initiate care within our services, serving as a liaison to Care Navigation.

You will collaborate with hospitals, physicians and case managers to ensure smooth transitions, complete initial assessments, obtain consents, and document according to policy; BLS is required, English fluency, and a willingness to travel for patient coverage.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience working with an EMR/EHR system.
• Strong clinical assessment skills; ED/ICU background helpful.
• English proficiency; Spanish bilingual preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete clinical admissions for all services and explain offerings empathetically.
• Identify barriers to admission and address objections promptly.
• Educate patients/families on admission requirements and next steps.
• Initiate care plans andNotify Physicians; document per policy.
• Coordinate with Care Navigation for timely follow-up and discharge.
