Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or hospital settings and to initiate care within our service lines. The role involves coordinating with Care Navigation, educating patients and families, and serving as a representative of the organization.

Requirements include an active RN license in Florida, at least 1 year of nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience working with an EMR/EHR system.

Responsibilities

• Complete clinical admissions for Company services.
• Educate patient/family on admission requirements and immediate needs.
• Coordinate with physicians and Care Navigation for care planning.
• Obtain consents and initiate Plan of Care; provide timely documentation.
