Chapters Health System in Florida is seeking an Admission RN to assess patients in their residence or in hospitals and initiate care with Care Navigation. You’ll serve as an ambassador for the organization and ensure a smooth transition into services.

The role requires an active RN license in the state, at least one year of nursing experience ( hospice/hospital preferred ), and experience with EMR/EHR systems. Spanish language skills are a plus and a valid driver’s license is required.

Formación

• Current RN license in the state of work.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; emergency department or ICU background preferred.
• Ability to communicate in English (verbal and written); Spanish preferred.

Responsabilidades

• Complete clinical admissions for all Chapters Health System services.
• Explain services to patients/families empathetically and address questions.
• Identify admission barriers and provide responses to facilitate admission.
• Educate patient/family on admission requirements and initiate Plan of Care.
• Obtain physician orders and start care plans; document per policy.
• Coordinate with Care Navigation and facilities for timely follow-up.
• Maintains professional representation of the Company at facilities visited.
