Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or in hospitals and initiate care within Chapters Health System. The role emphasizes a seamless transition to Care Navigation and serving as an ambassador of the Organization.

The position requires an RN license in Florida and at least 1 year of nursing experience; hospice or hospital background is preferred. A valid driver’s license and ability to lift patients as needed are required.

Qualifications

• Current RN license in the state where employed.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong assessment skills; ICU/ED background preferred.
• English communication; Spanish bilingual preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Completes clinical admissions for all services; assesses immediate needs.
• Explains services to patients/families with empathy and clarity.
• Educates on admission requirements and immediate next steps.
• Obtains physician orders and initiates plan of care accordingly.
• Communicates with Care Navigation to ensure timely follow-up.
• Provides care coordination and discharge planning as needed.
