Qualifications

• Current RN license in Iowa.
• Minimum two years’ nursing experience in long-term care or related setting.

• Proficient with electronic health records and MS Office.

Responsibilities

• Review referrals, medical records, and physician documentation for admissions.
• Assess clinical needs and determine facility’s ability to meet medical requirements.
• Coordinate admission nursing assessments and required clinical documentation.
• Facilitate communication between departments to prepare for incoming admissions.
• Support residents and families throughout the admission process.
