Job summary

Care Initiatives in Iowa is seeking an experienced Admissions RN to lead resident admissions, perform clinical assessments, documentation, and intake procedures, and serve as the primary clinical contact during transitions.

You will coordinate with CNAs, LPNs, and the interdisciplinary team to develop care plans, monitor conditions, and ensure compliance with state and federal regulations while maintaining compassionate resident care.

Qualifications

• Valid RN license in good standing; ability to activate license in Iowa.
• CPR certification on current, valid status.
• Experience in long-term care, skilled nursing, admissions, case management, or care coordination preferred.
• Knowledge of federal and state long-term care regulations and laws.
• Strong organizational, communication, and time management skills.

Responsibilities

• Lead and coordinate all resident admissions, including clinical assessments, documentation, and intake procedures.
• Serve as the primary clinical contact for admissions, families, hospitals, and referral partners.
• Review admission paperwork for accuracy and regulatory compliance.
• Complete admission assessments and develop resident care plans with the interdisciplinary team.
• Perform wound assessments, basic wound care, and clinical evaluations as needed.
• Assist as a charge nurse and with med pass as needed.
• Monitor resident conditions and respond to changes in status.
