Job summary

Great Lakes Management in St. Cloud, MN seeks a full-time Registered Nurse (Memory Care) to ensure resident health and safety through on-site visits, assessments, and clinical oversight. You will train staff and guide medical procedures to stay compliant with state and federal regulations.

The role requires professionalism, collaboration, and a team-focused approach when working with residents, families, and staff. Typical schedule is Monday–Friday with pay ranging from $36 to $40 per hour.

Qualifications

• Current RN license in Minnesota.
• Minimum one year of experience in home health, assisted living/Memory Care, or long-term care preferred.
• Strong organizational skills with the ability to self-start and follow through with minimal supervision.
• Ability to build positive working relationships with residents, families, and team members.

Responsibilities

• Assist and complete initial assessments of potential residents.
• Ensure residents meet admission criteria; reassess and recommend alternative placement when appropriate.
• Collaborate in developing and maintaining resident service agreements in compliance with state regulations.
• Monitor and maintain resident charts and clinical documentation.
• Communicate changes in resident conditions to physicians and county caseworkers as needed.
• Oversee medication management, renewals, and physician follow-up.
• Ensure proper medication administration procedures are followed and address corrections as necessary.
• Document medication disposal per company procedures.
• Provide initial and ongoing health training for staff; ensure all documentation meets regulatory standards.

RN Job Objective

The Registered Nurse (RN) in Memory Careensures the health and safety of all residents through on-site visits, assessments, and clinical oversight.
