The RN Float is responsible for assisting with the provision of care and services to clients receiving Assisted Living services, Assisted Living with memory care services, or services through the Good Shepherd Comprehensive Home Care license. Provides nursing leadership, direction, and supervisory oversight to licensed/unlicensed staff. Will be engaged in client pre-screening/admission process, developing a person-centered plan of care, and ongoing client assessments. Routinely engaged in ensuring plan of care for each client is up to date, processed, and communicated timely. Coordinates services ensuring coordination of care for each client and meets continued stay criteria. Provide direction to staff on engaged communication to clients with memory loss. Actively engaged in QAPI initiatives, develops best practices, assists with staff education, and ongoing policy development.
Assists area RNs in developing and updating a person-centered plan of care for each client after assessing/reassessing clients’ medical, nursing, and psycho-social needs by:
In conjunction with area RNs, monitors and directs all aspects of medication administration and/or self-administration.
Demonstrates a working knowledge of current nursing practices when performing observation, nursing assessments, clinical updates, post-fall follow-ups, and all mandated processes per regulation.
Demonstrates the ability to routinely assess and assimilate client needs and follow-up with appropriate nursing interventions, timely documentation, and staff training.
Coordinates and directs the provision of delegated nursing services to nursing/unlicensed staff in compliance with current licensure.
Directs and provides nursing supervision of licensed/unlicensed staff involved in the provision of Assisted Living Home Care services.
Will assist/engage in staff supervisory visits, evaluations, coaching, and disciplinary matters as needed.
Maintains ongoing awareness of state mandated and client services training needs.
Will be engaged in on-going training processes based on client needs, policy, and regulations.
Assists new nursing personnel to learn policies, procedures, and best practices.
Fosters respectful and engaged communication techniques with clients with memory loss and leads/directs staff in proper interventions with all clients.
Establish/maintain strong communications with families/caregivers and all agencies involved in client care including but not limited to: case managers, home care, hospice, pharmacy, care centers other Assisted Living’s, and any other health care professional to provide a continuity of client care.
Monitors and acts upon routine nursing data, notes, messages, and client/family calls to ensure client care plan reflects ever-changing needs.
Assist with direct care client services to ensure client plan of care is met.
Coordinates/arranges for needed services.
Practices and helps to maintain a safe environment for the client and staff.
Participates in RN On-Call process as assigned.
Fosters an environment that engages team building, safe discussion, questions, high levels of communication, etc. to ensure exceptional care and promote employee retention.
Communicates effectively and respectfully with all members of the team.
Actively participates in the agency Quality Assurance program. Works with supervisor to establish goals that will impact client care, promote Quality Assurance, foster improved staff education, etc. that help to fulfill the mission and values of Good Shepherd.
Engages in staff/team meetings and follow-up on assigned duties.
Completes mandated educational requirements and maintains licensing requirements for position.
Assumes responsibility for professional growth and development by actively participating in continuing education, in-service programs, orientations, and personnel evaluations.
