Caring Hospice Services is seeking a Registered Nurse Case Manager in Bucks County, PA. You will perform ongoing patient assessments and maintain thorough notes on each care plan, while remaining in constant contact with patient families and caregivers.

Responsibilities include pain management, supervision of Home Health Aides, and connecting families with community resources. A strong emphasis on confidentiality and coordinated care is essential.

Formación

• Pennsylvania RN license required.
• Bachelor's/Associate's/Master's in nursing required.
• Minimum 2 years in Med-Surg, Home Care or Hospice.
• Understanding of progression of declined vs custodial care.
• Ability to lift up to 25 lbs occasionally.

Job Summary

Qualifications And Skills

We are seeking an RN who is energetic, organized and emotionally capable of caring for terminally ill individuals. You must also be a team player, able to work harmoniously with a wide variety of individuals.

• Licensed as a Registered Nurse in the state of Pennsylvania.
• Associate/Bachelor's/Masters Degree in nursing.
• Have a minimum of 2 years Med-Surg., Home Care, and/or Hospice experience.
• Understand the difference between Progressively Declined and Custodial Care.
• Ability to occasionally lift up to 25 lbs.
