Job summary

Caring Hospice Services seeks a Registered Nurse Case Manager in Pennsylvania to perform ongoing patient assessments and keep thorough notes on each care plan. You will stay in close contact with patient families and caregivers to ensure comfort and effective symptom control.

The role requires an RN license, at least an associate/bachelor's/master's degree in nursing, and the ability to work independently in a hospice setting.

Qualifications

• Must hold a Nursing degree and PA RN license.

• Experience in Med-Surg, Home Care, or Hospice preferred.

About the Role

As a hospice Registered Nurse Case manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers.

Responsibilities

• Providing nursing care with an emphasis on pain and other management.
• Supervising Home Health Aides.
• Referring patients' families to applicable community resources.
• Maintaining strict patient confidentiality.
• Providing patient care directives to family members and alternative caregivers.

Requirements

Associate/Bachelor's/Masters Degree in nursing.

As a hospice Registered Nurse, you must have absolute integrity and sound judgment. You will act as a patient advocate, working independently to ensure your patients are physically comfortable and content. We are seeking an RN with good social skills, energetic, who is organized and emotionally capable of caring for terminally ill individuals. You must also be a team player, able to work harmoniously with a wide variety of individuals.

Additional Requirements

• Licensed as a Registered Nurse in the state of Pennsylvania.
• Associate/Bachelor's/Masters Degree in nursing.
• Have a minimum of 2 years Med-Surg., Home Care, and/or Hospice experience.
• Understand the difference between Progressively Declined and Custodial Care.
• Ability to occasionally lift up to 25 lbs.
• Bilingual (Spanish, Mandarin) a plus
