Job summary

Purposelink Recruitment is seeking a Registered Nurse Case Manager for Hospice in Pennsylvania. You will lead care coordination and supervise nursing staff in a community setting. This role requires an RN license in PA, hospice experience, and leadership abilities.

Local travel across multiple counties is required; schedule is Mon-Fri, 8:00 AM–6:00 PM with no weekend work unless desired. Mileage reimbursement may apply.

Qualifications

• 1+ year management/leadership experience.
• 1+ year experience in Home Health care (Home Health, Epic Home Health, or similar).

Registered Nurse Case Manager - Hospice | Pennsylvania | Full-Time

Overview

This is an opportunity for a compassionate and dedicated Registered Nurse Case Manager to join our client's hospice care team and make a meaningful impact in the lives of patients and families. You will work within a supportive interdisciplinary team, providing essential care coordination, leadership, and patient advocacy across the community.

Required Qualifications

Education & Experience

• High School Diploma or GED
• 1+ year of Hospice experience
• 1+ year of Management or leadership experience
• 1+ year of experience in ANY of the following:
  • Home Health Care
  • Epic Home Health
  • Home Health

Licensure & Certifications

• Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Additional Requirements

• Comfortable with local travel across multiple counties
• Reliable transportation
• Willing to work required hours; no weekends unless desired
