Purposelink Recruitment is seeking a Registered Nurse Case Manager – Hospice in Pennsylvania for a full-time role. You will provide patient care coordination, clinical leadership, and support to families navigating end-of-life care within an interdisciplinary team.

The position requires PA RN license, BLS/ACLS, at least 1 year hospice and 1 year management experience, willingness to travel locally (15–25 miles), and a Monday–Friday schedule 8:00 AM–6:00 PM with no weekends unless desired.

Qualifications

• High School Diploma or GED required.
• Minimum 1 year Hospice experience.
• Minimum 1 year management or leadership experience.
• Experience in Home Health, Epic Home Health, or Home Health is a plus.

Responsibilities

• Manage and oversee patient care in hospice settings.
• Develop, implement, and monitor individualized care plans.

Overview

This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join our client's hospice care team. In this role, you will provide essential patient care coordination, clinical leadership, and support to families navigating end‑of‑life care. You will work within a collaborative interdisciplinary team committed to delivering high‑quality, patient‑centered hospice services.

Required Qualifications

Education & Experience

• High School Diploma or GED
• 1+ year of Hospice experience
• 1+ year of Management or leadership experience
• 1+ year of experience in ANY of the following:
  • Home Health Care
  • Epic Home Health
  • Home Health

Licensure & Certifications

• Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Additional Requirements

• Comfortable with local travel across multiple counties
• Reliable transportation
• Willing to work required hours; no weekends unless desired

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs. The estimated hourly rate for this position is $80K
