Job summary

AndHealth LLC is seeking an Advanced Practice Provider (APP) with rheumatology experience to join our onsite care team in Indianapolis. The APP will diagnose, treat, and consult with patients, collaborating with a multidisciplinary team to deliver whole-person care.

You will develop personalized treatment plans, review progress with coaches, and participate in clinical rounds while leveraging telehealth and advanced EHR systems to document outcomes.

Qualifications

• Master’s degree from an accredited program.
• Certification as NP or PA-C required.

Responsibilities

• Create personalized treatment plans for patients and implement care plans.
• Review and feedback on treatment plans for Health Coach managed cases.
• Assess medication readiness and manage medications; order and interpret labs as needed.
• Collaborate with internal/external providers to ensure access to care needed to reverse chronic diseases.
• Participate in evolving model of care and measure effectiveness.
