Job summary

AndHealth is seeking an Advanced Practice Provider with medical dermatology experience to join our onsite care team. The APP will contribute to diagnosis, treatment planning, and care coordination alongside health system partners.

Ideal candidates hold IL PA-C or NP license, Master’s degree, and experience with telehealth, EHRs, and collaborative care models. This full-time onsite role includes competitive benefits and opportunities to innovate care delivery.

Qualifications

• Active license to practice as PA or NP in Illinois.
• Certified Nurse Practitioner (NP) or PA-C certification required.
• Master’s degree from an accredited program.

Responsibilities

• Create personalized treatment plans and implement care with patients.
• Support clinical treatment protocol updates by AndHealth team.
• Review Health Coach cases and provide feedback on plans.
• Assess medication readiness and manage medications; order/interprete labs.
• Oversee patient progress; coordinate care with coaches; act as escalation point.
• Collaborate with internal and external providers to ensure access to care.
• Participate in innovating and measuring the model of care.
• Other responsibilities as necessary to meet business needs.
