Job summary

AndHealth LLC is seeking an Advanced Practice Provider (NP or PA-C) with rheumatology expertise to join our on-site care team. You will develop and implement personalized treatment plans, work with Health Coaches, and coordinate care with internal and external providers to reverse chronic diseases.

You will leverage detailed protocols, maintain strong patient relationships via telehealth, and participate in continuous improvement of our care model while meeting evolving business needs.

Qualifications

• Licensed and certified Advanced Practice Provider (NP or PA-C) with rheumatology experience.
• Master’s degree from an accredited program.
• Proven ability to work within a collaborative care team.
• Strong telehealth experience and patient relationship management.

Responsibilities

• Create personalized treatment plans and implement care with patients.
• Follow detailed clinical treatment protocols.
• Review Health Coach cases and provide feedback on plans.
• Assess medication readiness and order labs as needed.
• Coordinate care with internal and external providers and participate in clinical rounds.
