AFCAMP Family Empowerment Center, Inc. is seeking a R&TFT Counselor to support families whose children have been in foster care reunify and thrive.

You will assess readiness, provide guidance during supervised visitation, parenting-skill building, and coordinate care with providers. The role requires delivering case management, counseling, and crisis support in office and community settings, with a flexible schedule and responsibilities across planning, documentation, and collaboration with team

Responsibilities

• Develops case management plans or plans of care and provides direct case management services and/or coordination of care for each client/family.
• Provides front line crisis support to individuals and/or families by phone, in-home and/or in the community.
• Makes decisions based on interpretation of data and adapts program guidelines to client needs.
• Determines processes to follow based on assessment and client strengths; takes action to support family.
• Provides group, individual and/or family counseling with supervision and coordinates treatment phases with providers.
• Maintains records of counseling, coordination of care, team meetings and case management activities.
• Demonstrates knowledge of psychopathology, crisis theory and intervention modalities relevant to target population.
• Supports daily program operations; may transport clients to appointments.
• Provides services in office and community settings and maintains a flexible schedule.
• Provides safety planning and crisis intervention with supervision including on-call coverage to ensure 24-hour availability.
• Participates in developing and facilitating treatment plans and transition plans.
• Identifies referral processes and makes clinically appropriate referrals.
• Meets direct service productivity goals and adheres to guidelines and reporting.
• Identifies risk factors and takes measures to reduce risks in the home.
• Attends supervision and meetings to coordinate care and review treatment plans.
• Communicates effectively with client, family and providers.
• Continues to develop knowledge about culture and values of groups served and uses appropriate approaches.
• Regulatory and Compliance Responsibilities: understands and adheres to compliance standards and completes required trainings.
• Performs other related duties as assigned.
