AFCAMP Family Empowerment Center, Inc. seeks a dedicated R&TFT Counselor to support families reunifying after foster care.

The role provides case management, guidance during supervised visitation, and parenting-skill building to promote child and family wellbeing. You will work directly with parents and children, coordinating services with internal and external providers, maintaining documentation, and ensuring compliance.

Responsabilités

• 1. Develops case management plans and provides direct case management services or coordination of care for each client/family, including interventions to increase treatment completion and life functioning.
• 2. Provides crisis support to individuals/families by phone, in-home, or in the community; interprets guidelines and consults with supervisor.
• 3. Makes decisions based on data; adapts guidelines to client needs.
• 4. Determines processes and actions based on assessment and strengths.
• 5. Provides group/individual/family counseling with supervision; coordinates treatment with providers.
• 6. Provides counseling and coordinates treatment with external supports as appropriate.
• 7. Maintains records for reporting and regulatory compliance.
• 8. Demonstrates knowledge of psychopathology, crisis theory, and intervention modalities relevant to the population.
• 9. Supports daily operations and may transport clients to appointments.
• 10. Delivers services in office/community settings with a flexible schedule.
• 11. Supports safety planning and crisis intervention with on-call coverage.
• 12. Develops transition plans and timelines for treatment strategies.
• 13. Refers to internal/external services as needed.
• 14. Meets productivity goals and adheres to guidelines and reporting protocols.
• 15. Identifies risk factors and applies trauma-informed interventions.
• 16. Attends supervision and meetings; seeks guidance when needed.
• 17. Communicates effectively with clients and providers.
• 18. Develops knowledge of cultures and uses appropriate methods.
• 19. Adheres to compliance plans; completes required trainings.
• 20. Performs other related duties as assigned.
