Job summary

Qualifications

• RN licensure in the state of WV
• BLS/CPR certification
• Experience in long-term care, post-acute care, rehab, or geriatric nursing preferred
• English proficiency for documentation and patient communication

Responsibilities

• Perform comprehensive clinical assessments and develop care plans.
• Supervise and delegate to LPNs and CNAs.
• Coordinate interdisciplinary care and discharge planning.
• Maintain accurate PCC documentation and ensure patient safety.
• Collaborate with physicians and families to optimize outcomes
