Job summary

Genesis HealthCare in West Virginia is seeking a Registered Nurse to provide patient-centered care across a range of diagnoses in our long-term care setting. You will perform nursing assessments, develop individualized care plans, and collaborate with the nursing team and families to promote residents’ well-being.

The role includes administering medications, delegating tasks, and ensuring seamless communication across shifts to improve outcomes while minimizing rehospitalizations.

Qualifications

• Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
• CPR Certification is required.

Responsibilities

• Perform nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to develop effective plans of care.
• Establish realistic, measurable short- and long-term patient goals with interventions to minimize rehospitalizations and to evaluate outcomes.
• Administer medications and perform treatments per physician orders.
• Delegate patient care responsibilities to staff and supervise them to ensure that tasks are implemented.
• Communicate patient information with assigned staff and between shifts.
