Job summary

Aperion Care Oak Lawn seeks an MDS Assistant/Restorative Nurse (RN or LPN) to lead the restorative nursing program, coordinating between therapy and nursing to promote restorative care across the facility.

Responsibilities include developing and monitoring the restorative plan, conducting assessments, supervising staff, attending care planning and QAPI meetings, ensuring documentation, and acting as Medicare Services Coordinator when needed.

Qualifications

• 3–5 years of Restorative Nursing experience.
• Current Restorative Nurse certification preferred where allowed by state regulations.
• Incumbent must be a nurse currently registered with the State Board of Nursing.
• Current CPR certification is required.

Responsibilities

• Develop, organize and implement restorative nursing philosophy and standards.
• Assess quality of nursing care and recommend improvements.
• Perform/oversee restorative assessments and develop care plans.
• Coordinate with therapy to ensure recommendations are carried through.
• Attend care planning conferences and QAPI meetings.
