Job summary

The Alden Network is seeking a Restorative Nurse to lead development, implementation and monitoring of the facility's restorative nursing program. You will coordinate between therapy and nursing to promote residents’ highest functional level.

You will maintain restorative plans, monitor progress, and ensure documentation accuracy across units while collaborating with nursing, dietary and therapy teams.

Qualifications

• Must possess a current, unencumbered license to practice as an RN or an LPN in the state of Illinois.
• Must be restorative certified by an accredited school or recognized accrediting agency.
• Must possess current CPR certification or become certified within two weeks of employment.
• Must be able to read, write and speak the English language in an understandable manner.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct restorative care services, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern nursing care facilities.
• Maintains a current list of customers participating in a restorative program on each unit with unit-specific details.
• Sets up documentation flow sheets and electronic medical record system for daily documentation of restorative nursing measures.
• Makes quarterly progress notes and ensures all restorative care notes describe treatment and response.
• Oversees restorative nursing measures reflected on the care plan and coordination with therapy, nursing, and dietary to obtain devices as recommended.
• Works with Therapy department to ensure discharge recommendations are carried through.
• Performs or oversees functional assessments on admission and quarterly; collaborates with nurses on restorative plan of care.
• Oversees customer ADL performance and CNA documentation for accuracy and completeness.
• Oversees continence management programs and participates in care planning conferences.
• Completes sections of MDS and CAA summaries and tracks quarterly quality indicators.
