Job summary

Aperion Care is seeking a MDS Assistant/Restorative Nurse to back up the MDS Coordinator and oversee the restorative nursing program across the facility. You will lead development, implementation, and monitoring of restorative care, acting as a bridge between nursing and therapy to promote a patient-centered approach.

You will collaborate with the Director of Nursing, participate in care planning, and supervise staff to maintain compliance and high-quality resident care.

Qualifications

• 3–5 years of restorative nursing experience required.
• Current Restorative Nurse certification preferred.
• RN license in state of Illinois.
• Current CPR certification required.

Responsibilities

• Assist in development and implementation of restorative nursing program.
• Monitor and supervise restorative nursing staff and documentation.
• Coordinate with therapy and nursing to ensure continuity of care.
