Job summary

LifeSpan Therapy in Indiana is seeking a Restorative Certified Nursing Assistant (CNA) to support residents' independence through restorative care and collaboration with the therapy team. This role emphasizes compassionate, person-centered care and aims to enhance mobility and daily living activities.

You will implement individualized care plans, monitor progress, document changes in the clinical record, and respond to residents' needs promptly.

Qualifications

• Current Indiana CNA certification is required.
• Experience in resident care within healthcare or long-term care is preferred.
• Ability to use basic computer applications for documentation and communication.

Responsibilities

• Delivers quality care with kindness and respect for resident dignity.
• Receives and completes patient care assignments from Therapy Supervisor.
• Provides direct care to maintain or improve residents' abilities per care plans.
• Protects residents' rights, privacy, and confidentiality.
• Responds promptly to call lights and emergencies.

• Monitors changes in residents' psychosocial status and reports to Charge Nurse.
• Follows infection control procedures; adheres to facility policies.

About the Role

The Restorative Certified Nursing Assistant (CNA) at LifeSpan Therapy plays a vital role in enhancing the quality of life for residents by promoting their independence and functional abilities. This position focuses on providing specialized restorative care that supports res i d ents in regaining and maintaining their physical capabilities through consistent, compassionate assistance . The Restorative CNA collaborates closely with nursing staff and therapists to implement individualized care plans that address mobility, strength, and daily living activities. By monitoring patient progress and reporting changes, the Restorative CNA ensures timely interventions that contribute to improved health outcomes. Ultimately, this role is essential in fostering a supportive environment that encourages res i d ent dignity, safety, and well-being.

Job Responsibilities

• Delivers quality care in a compassionate manner with kindness and respect for resident dignity and individuality.
• Receives and completes all patient care assignments and other activities assigned by the Therapy Supervisor.
• Provides direct care to improve or maintain a resident's abilities according to the resident's plan of care.
• Provides care in a manner that protects and promotes resident rights, dignity, privacy, confidentiality, self- determination and active participation.
• Responds to call lights/buzzers promptly (within 5 minutes of first ring), courteously treating each as an emergency until answered.
• Records accurate , legible information about resident care, conditions, and observations in the appropriate locations in the clinical record.
• Observes and immediately reports to the Charge Nurse and Therapy Supervisor significant changes in a resident's psychosocial behaviors such as patterns of decreased social interaction, angry or sad behaviors, and refusals of care or treatment.
• Always follows infection control procedures.

Minimum Qualifications

• Current Certified Nursing Assistant (CNA) certification in the state of Indiana .
• Demonstrated experience in res i d ent care within a healthcare or long-term care setting.
• Ability to use basic computer applications for documentation and communication.

Preferred Qualifications

• Previous experience specifically in restorative nursing or rehabilitation care.
• Familiarity with electronic health record (EHR) systems.
• Strong interpersonal skills with the ability to motivate and encourage patients.
• Experience working in a multidisciplinary healthcare team environment.
